Fantasy anime have always transported us to another immersive world, but are they actually faithful to the source? The original concept is the author's vision for what the story is and potentially can be.

Changing these types of things weakens the overall story and even the pacing. Unnecessary fillers lessen the emotional impact that scenes give, while sometimes it does give the slow burn, other times it just kills the story and makes it a snoozefest.
Fullmetal Alchemist: Brotherhood

Fullmetal Alchemist: Brotherhood is the 2009 completed anime which is regarded by many as one of the greatest anime of all time. Me personally, I think it's great, but greatest of all time? Not so much. Still, it's definitely up there with really good anime.
This story follows two young alchemist brothers, Edward and Alphonse Elric, who live in a world where alchemy is a powerful science. After trying to bring their mother back from the dead using forbidden human transmutation, the experiment goes horribly wrong. Edward loses an arm and a leg in the process, while Alphonse, the youngest brother, loses his entire body and has his soul bound to a metal suit of armor. This sets them on a journey to find the Philosopher's Stone to restore their lost body parts.
This specific one, Brotherhood, is considered the more faithful adaptation because unlike the 2003 version which had to invent its own story midway through, Brotherhood closely aligns with Hiromu Arakawa’s work, the original manga creator. This one keeps the very core plot, all the major twists, and more specifically the character development, which is by far the most compelling thing in the story for me. The bond between the brothers is what I think hooked everyone.
Mushishi

Mushishi is one of the more calm fantasy anime that follows Ginko, who is a wandering Mushishi. This means he is a specialist who deals with Mushi, mysterious spirit creatures that are invisible to most humans. In each episode, Ginko travels through the rural areas of Japan, helping villagers and solving problems.
This series is one of the most faithful anime adaptations ever made because it follows the original manga creator’s work to a T. There is almost no filler at all and no changes to the story whatsoever. The most important thing also is that there are almost no cuts. This anime perfectly captures the creator’s vision, and even the visual style and slow pacing are kept identical to the manga. Personally if you’re looking for a one to one adaptation, this series is your best option. It’s a wonderful story overall and a laid back one where you can just shut your brain off and watch. The only down side is that this is one of those one and done anime, where its hard to rewatch.

Frieren: Beyond Journey’s End

This one will come as no surprise. Frieren is a beautiful emotional fantasy anime that is very faithful to the source. This story follows an elf girl who goes through centuries of life slowly learning about humanity, forming new bonds, and reflecting on the meaning of time, loss, and the little moments in life. It’s more of a chilled, laid-back series and honestly there’s a reason it’s rated so high on places like MyAnimeList.
This anime adaptation closely follows the manga panel for panel. This not only makes the story more captivating but also captures the author’s vision. Of course, every anime that follows this pattern no doubt becomes widely acclaimed. Very few scenes were cut and the accurate manga-to-anime visuals make this a standout hit. So if you haven’t watched it, I highly recommend this one.
The Apothecary Diaries

The Apothecary Diaries isn't purely fantasy but it's a clever historical mystery mixed with some fantasy elements. It follows Maomao, who is a sharp witted young girl with knowledge of herbs and poisons. After being sold as a servant into the emperor’s inner palace, she quietly uses her skills to solve strange incidents.
This is definitely one of my top anime. At first it doesn't seem that interesting, but as you go deeper into the story, Maomao’s bubbly personality and genius make the story compelling and hook you from beginning to end. This series is highly faithful to its source, which is the light novel by Natsu Hyūga. The anime has very few changes and keeps the same dialogue and humor, which are by far the strengths of this series. The slow burn storytelling is what hooks you. Now merge that with no unnecessary filler and you have a masterpiece.
Spice and Wolf

Spice and Wolf follows the adventures of a traveling merchant named Kraft Lawrence who meets an ancient deity fox named Holo who takes the form of a wolf girl with a tail. Together they journey across this medieval fantasy world and come up with clever schemes till they reach Holo’s homeland in the north.
This adaptation is considered extremely faithful to the light novel series by Isuna Hasekura, and the 2024 remake is considered even more faithful than the original one. This new remake adapts the skipped content that the original anime skipped which restores the original events, and to be honest it keeps you invested. While the original was good and even widely considered a success, this new remake definitely surpasses it.

The Ancient Magus’ Bride

The Ancient Magus' Bride is a Japanese fantasy series about Chise Hatori who is a lonely 15 year old orphan who has a tragic past. However, everything changes when she is sold at an auction in London. She was bought by Elias for an enormous amount of money. Elias takes Chise as both his apprentice and future bride and introduces her to the beautiful hidden world of magic and all fantasy creatures.
The adaptation which was animated by WIT Studio is incredibly faithful to the source and preserves the world-building and all the folklore-type elements. A faithful adaptation for this series is incredibly important for the romance part because it creates a slow burn that pulls you in even more.
Bonus Recommendation
Violet Evergarden

Violet Evergarden is a Japanese anime adapted from the light novel by Kana Akatsuki and follows a girl named Violet Evergarden who was once a weapon used as a child soldier during war times. However, now that the war has ended she joins the Auto Memory Doll service which is a professional letter writers. Now Violet travels across the world writing letters for others while slowly healing from the bad experiences during the war.
So this series is considered loosely faithful but I had to get this one in here as more of a bonus because it is that good and while it is loosely faithful because they arranged the chapters in a more linear character focused narrative however the original author Kana Akatsuki was heavily involved and supportive during the production of the anime which kind of officially makes it faithful right? and she even praised the adaptation.
